ICAMA System & Forms

The New ICAMA SYSTEM on the eCare Vault (eCV) platform is up and running!

Thank you to all state staff and administrators who helped to make this transition possible.

State Staff who are Users of the ICAMA System may now access the eCare Vault platform by:

  1. Submitting a completed, signed and dated User Policy Agreement to the National Office.
         Note! California Users must contact their Compact Administrator to obtain the California User Policy Agreement.
  2. Logging on to www.my.ecarevault.com
  3. Creating your account using the information you provided on your User Policy Agreement
  4. Accessing resources such as:

ICAMA System User Policy Agreement Please contact the National Office if you are unable to download or have questions about its use.
This form is required to access the ICAMA System on the eCare Vault platform. It must be signed by a state Compact or Deputy Compact Administrator or Supervisor and returned to the National Office before you can access the new ICAMA System.

ICAMA Add/Disable User ID Form rev2022
This form is used and is still required* to request that an ICAMA System User be added or removed (disabled) from the ICAMA System of electronic transfer of ICAMA cases.   Note: This form is fill-able.  * Note: this is a separate form from the User Policy Agreement.

ICAMA Designation of Compact Official Form rev2021
This form is used to list each state’s Compact and Deputy Compact Official(s). These individuals are authorized by their state to represent the state in operation of the Compact and Association business to include Board membership and voting rights. Note: This form has been updated and is fill-able.

ICAMA_7.01 California and New York rev2022  — Please contact the National Office if you are unable to download or have questions about its use.
This form is the ICAMA System referral/request used to open Medicaid for the states of California and New York. Revision date: 12/31/2022.  Note: This form is fill-able. This Form was previously titled: ICAMA_7.01 CA NY OpenMed.

ICAMA_7.5 CA NY Cases Opened with ICAMA Form 6.01 or 7.01 rev2022Please contact the National Office if you are unable to download or have questions about its use.
This form is used to make changes to all cases with California or New York or any case opened before 2015 and used to Add Children to all cases with California or New York or any case opened before 2015 (via the ICAMA Form 6.01 that was in use prior to the ICAMA System of electronic transfer of ICAMA cases.)  Revision date: 1/31/2022    Note: This form is fill-able. This Form was previously titled:  ICAMA_7.5 CA NY Information Exchange.

ICAMA 700FORM- For Use as a BackUp to eCareVault Platform Please contact the National Office if you are unable to download or have questions about its use.
This form is the ICAMA System referral/request used to open Medicaid during the new ICAMA System conversion – For use by all states except California and New York.  See form below: ICAMA_7.01 California and New York rev2022

 

The Non-Title IV-E Guardianship Information Transfer Form can be used to transfer information from an Agreement State (state with which the family holds a Non-Title IV-E Guardianship Agreement) to a Resident State (state where the family currently resides.)

Note: This form is provided as a courtesy to support state administration of children’s permanency. Guardianship Agreements are not under the jurisdiction of the Interstate Compact on Adoption and Medical Assistance (ICAMA.)

ICAMA_7.02b Info for Families rev2017
This form is used to make changes to all cases with California or New York or any case opened before 2015 (via the ICAMA Form 6.01 that was in use prior to the ICAMA System of electronic transfer of ICAMA cases.)   Note: This form is fill-able.

ICAMA_7.5 CA NY Additional Children rev2017
This form was subsumed into the ICAMA_7.5 CA NY Information Exchange rev2022 and was used as a companion form that accompanied the ICAMA Form – 7.5 Info Exchange.  This form was used to add children to all cases with California or New York or any case opened before 2015 (via the ICAMA Form 6.01 that was in use prior to the ICAMA System of electronic transfer of ICAMA cases.)   Note: This form is no longer in use.

ICAMA_7.5b_Explanatory_Info rev2017 – Information for Professionals
This form is an informational document for ICAMA workers regarding form terminology, assistance programs, and federal law.
